IN CASE OF DEATH OF
RICHARD CONRAD CLARKE
1. Obtain at least 10 copies of Death Certificate, to be submitted as follows:
A. Public Employees Retirement System (PERS), 1416 Ninth Street, P. O. Box 1953, Sacramento, Calif. 95809.
Telephone: (916) 326-3848.
Give them the following data:
Notify PERS of Death and ask that retirement checks be mailed to F. DeVonne Clarke - direct to Bank of America checking Account No. 06686-03452 in Westlake Village - same as has been done for Richard C. Clarke. It may take 2 or 3 months to get it done (They are very slow). Meanwhile use money from other accounts. Amount of check each month should be the same for DeVonne as it has been for Richard.
NOTE: There is a death benefit of $2,000.00 to be paid to DeVonne. (Should be paid immediately upon their receipt of Death Certificate.)
Also notify University of California Benefit Programs, 2111 Bancroft Way, Berkeley, Calif. 94720.

Also phone UCLA Retirement Office and notify them of Death. Let them know PERS (above) has been notify by mail. UCLA Retirement Office Telephone No. is (213) 825-4353. (May not be a good number anymore. Okay 9/30/88)
NOTE: Health Insurance with Kaiser and Dental Care with Delta Dental Plan of California (Calif. Dental Service) will continue for DeVonne after death of Richard.
NOTE: All health work must be done by Kaiser except in emergency. Anything done outside of Kaiser will not be paid for by Medicare. Must get to hospital at Woodland Hills or use clinic in Newbury Park when applicable.
B. Veterans Administration
Notify VA of Death. Insurance File No. and Policy No. is V 1226 64 44
Amount: $5,000.00 payable to DeVonne. (If DeVonne is dead $5,000.00 is split equally between sons, Dicky, Reggie & daughter, Kandy.)
Premium is $15.20 paid monthly by automatic transfer from Bank of America checking account in Westlake Village. Stop payment after death. Refund may even be available depending upon date of death and time of last payment.
VA Policy is in Safety Deposit Box Y1933 in Bank of America Westlake Village Branch. (Submittal of Death Certificate with letter giving Policy No. for identification should be all the VA needs to send check for $5,000.00.

C. Social Security
Keep first check received after death. (Check is mailed directly to Bank of America Checking Account in Westlake Village Branch.) All subsequent checks should be returned to Social Security Office. Ask Bank how to handle this as check is mailed directly to them. (I don't think they even send a check, it may be another computerized transmittal.)
NOTE: The Social Security amount DeVonne is receiving at time of death of Richard should be adjusted by the Social Security Administration.
D. Military
Mortuary should also be able to tell who to notify in the Military of death. (Notification to Veterans Administration may be enough.) There may be other Military death benefits.
E. Exxon Company, P. O. Box 3505, Houston, Texas 77001
In case of death in Auto Accident (also may include Aircraft) notify Exxon. Insurance Policy is in Safety Deposit Box No. Y1933 at Bank of America Westlake Village Branch.
Policy pays $20,000.00 in case of death. We are making monthly payments of $2.75 on Exxon Gasoline billing. Coverage is for both DeVonne and Richard.
F. Auto & House Insurance is with State Farm Insurance Co.
2. Financial Accounts for Richard & DeVonne are in the following Mutual Funds:
The amounts in the above accounts will vary from time to time but normally there will be a total amount in all the accounts of approximately $30,000.00.

4. Coin Collection

Purchase prices are shown on listing kept with coins. Most coins are Silver, some Gold, and some of non-precious metals.
To dispose of coins contact a reputable coin dealer for best method of selling. (Should get at least what was paid for them.) Should sell only when Gold is $600.00/ounce and Silver is $20.00/ounce, or higher.
I would prefer coins to be left to Kandy to hold for one of her children that she will find interested enough to keep them. The longer they are held the more valuable they become, usually.

Monthly payment is fixed at $320.00 per month. Bank is authorized to transfer $320.00 on the 10th of each month from Savings Account No. 0-129792-8 to Loan Account No. 0-922245-6.
Must deposit money to Checking & Savings Account regularly (monthly) to cover transfer and checks written for such items as: Home Insurance, Auto Insurance, Property Taxes, Etc.
